Students finding it necessary to write a final exam at a time other than the scheduled final exam must submit this form no later than the posted deadline posted in the Academic Calendar. If you do not submit this form by the deadline, you will be required to write your exam(s) during the previously scheduled time or accept the academic penalties noted in your syllabi. Requests for a revised examination time will only be considered for the following two reasons:
